International links and research: With my colleague in pediatrics neurology

  1. In collaboration with Victor Pattrision a pioneer in teleneurology at UK and visiting professor at University of Khartoum-faculty of Medicine we start a project to Develop  an epilepsy application that help distinguish epileptic versus non epileptic events in resource limiting countries
  2.   In collaboration with department of biochemistry at faculty of medicine and Neuroscience University of Paris 6 we establish the neurogenic project with especial interest in spinocebellar ataxia, epilepsy, autism, and Parkinsonism and Neurodegerative disorders among Sudanese families.
  3. In collaboration with Kruer Lab- Phoenix Children’s Hospital- University of Arizona, College of Medicine, Phoenix, AZ we start the genetic cerebral palsy projects.

 New establishment: –

  • Paediatric neurology clinics- Soba University Hospital – 2007

With my colleague, this clinic was the first paediatric clinic devoted for children with neurological disorders in Sudan. It started at Saad Abo Alellah University hospital and in2010 relocated at soba hospital. It covers 60-80 patients per week/ clinic. Patients referred from all over Sudan and even from neighbouring countries eg Chad and Eritrea. A lot of fund raised for this clinic and we manage to make all antiepileptic drugs available free now the total number of patients in this clinic exceeded 20.000.  

  • Paediatric Neurology subspecialty unit at Gafer Ibn Auf Hospital- 2016:

 Established at Gafer Ibn Auf Specialised Hospital for Children- Khartoum in 2010. This unit was the first paediatric neurology unit in Sudan; it covers all variety of neurological disorders with acute neurological problems. It was started as 12 beds ward and now as the admission exceeded the bed capacity we establish a new ward with extra 8 beds and 4 beds as high dependency unit (HDU) In addition we cover patients admitted to Intensive Care Unit (ICU) and consultations form other subspecialty units.

  • Outpatient Department for epilepsy and neurology at Gafer Ibn Auf Specialized Outpatient Clinic at Fath Alrhman Elbasher Centre(serve as gafer Ibn Auf OPD) 2017

This clinic was the first paediatric clinic devoted for children with neurological disorders in Sudan following Soba OPD. It covers 60-80 patients per week/ clinic.   • Psychology clinic / IQ measurement clinic at Gafer Ibn Auf Specialized Hospital for Children 2018

This clinic started at 2017 by workshop training for five psychologists on Benet5 IQ measurement Arabic version. We manage to avail three sets Benet5 IQ measurement. The clinic cover 5-7 patient per clinic

  • Epilepsy clinic at Gafer Ibn auf Specialized Hospital for Children 2018

This clinic established as the number of patients with seizure disorders are growing very fast and exceed the capacity of the general neurology clinic. It conducted as once per week with availability of AEDs free. As well, we managed to have an EEG machine and to recruit junior specialist with special interest in epilepsy to work in this clinic. We manage to have detailed written protocols for management of childhood epilepsy.

  • Transitional clinic for epilepsy – Gafer Ibn Auf Specialized Hospital for Children 2019:

This clinic established as a lot of our patients are now approaching adolescent and adult age. The referral to adult service is not ease and rather chaotic. In addition these [patients at this age needs are different and need different approach. The clinic is joined with adult neurology (fellowship students) It is once/ month and it cover around 20-25 patients per clinic. Patients have free access to antiepileptic drugs.

  • Pediatric Epilepsy Training Course (PET1) in collaboration with British Pediatric Neurology Association. 2015:

With my colleague, we manage to have (for the first time in Africa and the region) the Pediatric Epilepsy Training course (PET1) course – British Paediatrics Association – Soba Exam Centre 2015 – 2019. The Sudanese charity Sadagaat, agreed to fund a 5-year programme to run PET courses annually. The course delivered by an experienced faculty with Sudanese heritage now working in the Gulf States, 2 UK faculty members and myself and Ahlam Ahmed Hamed, Members of the ILAE paediatric commission, Jo Wilmshurst and Patrick van Bogaert, who were in Khartoum to speak at the joint Sudan-ICNA conference, observed the course.  This reported in the BPNA annual report2014-2015 and in the Sudanese journal of paediatrician. Now we trained almost 400 pediatric consultant/ specialist with considerable number from outside Khartoum State. PET aimed at pediatricians, medical officers and emergency department professionals. It aims to improve the diagnosis of epileptic and non-epileptic events; improve the standard of care; and raise awareness of when to liaise with a Paediatrics Neurologist, a children’s epilepsy expert.

With my colleague we manage to have (for the first time in Africa and the region) the  Pediatric Epilepsy Training course (PET2) course – British Paediatrics Association – Soba Exam Centre 2017 – 2019

  • Children’ Epilepsy Diagnosing Aid(CEDA) Application. With collaboration with Dr. Victor Pattrision a visiting professor at university of Khartoum we are able to develop a Children’ Epilepsy Diagnosing Aid(CEDA) Application. The app will be for non-specialist doctors and non-physician healthcare workers to use at PHC level .This tool is likely to be useful in other LMICs where the problems for children with epilepsy are likely to be similar to Sudan. Translation is possible into other languages .

MRCPCH Examiner Elect Course  2016

With my colleague at department of pediatrics  we manage to host the first MRCPCH Examiner Elect Course and the first MRCPCH Mock Clinical Exam at Soba Education, Training & Examinations Centre (SETEC), Soba University Hospital, on the 27th and 28th of September, 2016.

  Preparatory courses for MRCPCH:

With other local faculty we manage to run courses to prepare local candidates on communication skills, history taking, developmental examination, and other needed clinical skills. Started in 2017- 2019.

Out reached clinic:

  • Ombaddah locality: Sudanese Cerebral Palsy association among other activities including talk to publics arranges the clinic.  The clinic was conducted at Ombada Higher School for Girls, total number of patients seen is 182 the majority had neurodisabilties and / or epilepsy, 45% of the patients never seek medical advice. Follow up visit arranged at Fath Elrahman Elbasheer OPD. Investigations and treatment provided.   (2018).
  • Al-Gaili Area: Sudanese Cerebral Palsy association arranges the clinic. The clinic was conducted at Algili hospital, total number of patients seen is 100 the majority had neurodisabilties, down syndrome and epilepsy, 20% of the patients never seek medical advice. Follow up visit arranged at Fath Elrahman Elbasheer OPD. Investigations and treatment provided.  (2018).
  • Jabal Awlia locality: The clinic arranged by Nafeeer charity organization.  The clinic was conducted at Gabal Awilia hospital, total number of patients seen is 500 the majority had neurodisabilties and epilepsy, 15% of the patients never seek medical advice. Follow up visit arranged at Fath Elrahman Elbasheer OPD. Investigations and treatment provided.   (2017)
  • Al-Samrab area: The clinic was arranged by ToGether charity organization.  The clinic was conducted at Al-Samrab Youth , total number of patients seen is 150 the majority had neurodisabilties, autism spectrum disorders  and epilepsy, 58% of the patients never seek medical advice. Follow up visit arranged at Fath Elrahman Elbasheer OPD. Investigations and treatment provided.   (2017)
  • Organization of fun days (entertainment) for inpatient children at Gafer Ibn Auf Specialized Hospital during EID times.
  • With my colleague and leadership at ministry of health, we lead an Advocacy towards free antiepileptic drugs, and we manage to make AEDs available for children with epilepsy all over Sudan and for all ages free.

Network support with parents of children with epilepsy and neurodisabilties 

Some of our patients, especially those in States other than Khartoum, frequently get shortage of medications due to either unavailability or unaffordability and other medical concerns that need urgent help. That is why we thought of using WhatsApp to communicate with caregivers of children with neurodisabilties and epilepsy and to communicate with their local doctors as well which will decrease unnecessary travel to Khartoum in the context of difficult and costly transportation.

During Covid 19 we manage to run the neurology consultation and epilepsy clinic using telephone calls, telegram and WhatsApp, which had great impacts on patient’s health especially during this difficult time taking into consideration the financial issues.

Contribution to medical education

  • I have been among the first Batch enrolled in the Master degree in health professional education at EDC- faculty of medicine – University of Khartoum. I have been among the first facilitators group contributing to the  teaching of  Batch 2 in medical education
  • As a director of the EDC, we have graduated almost 90 graduates in medical education, of whom almost half of them are belonging to the medical campus – university of Khartoum. This group of graduates has a very strong effect on the quality of medical education in their faculties.
  • I am supervising the program of master degree of health professional. The program involved candidates from other governmental and private universities eg Islamic University, Karary University, National Ribat University, International African University and Ahfad for Women University plus others. This had led to excellence in service delivered to the community by graduating competent health personnel who are well taught and trained to cope with the expanding numbers of teaching institutes in medicine and health sciences.

The graduates of this master made the nidus for professionalism courses and curriculum development and reform committees at SMSB, Student support unit at faculty of medicine, establishment of educational development centers for other universities.

  • We establish strong relationship with regional and international bodies such as the Medical Education unit at Dundee University in UK and Association of Medical Education in Europe (AMEE). They over our candidates 50% discount when they apply to master programs in Dundee. They over online video conferences and keynote lectures for our master students.
  • In collaboration with Soba university hospital we manage to train up to now 100 nurse in patient critical care – each course is 6 months duration and we manage to do this course free. This had its effect on quality of care at the level of Emergency room, ICU and HDU and hence improving the overall health.
  • We conducted a number of basic life support courses and first AIDs to non- medical personnel eg workers in petroleum companies and traffic police officers. Some of these courses are conducted in remote areas eg Hijlij. This training has its impact on doing lifesaving procedure while awaiting medical help, which will result in good outcome and saving life.
  • In collaboration with medical students union, we establish mentorship program aiming to promote healthy learning environment, sharing experience, skills, knowledge, and foster culture for development. In addition to graduate students who are academically excellent, self-developed, goal directed and career oriented.
  • I organize and facilitate a number of workshops targeting medical students and junior doctors eg Career Pathway, time management and dealing with stress.
  • I organized, and facilitate number of courses and workshops targeting staff continuous development.
  • I appointed as co- supervisor from Department of Medical Education, University of Dundee.

During Covid 19 lock down

  • We establish strong relations with our Sudanese colleague who have excellent experience in medical education in the Gulf, UK, USA to facilitate webinars addressing different issues in medical education and this was extremely helpful.
  • During Covid 19 lock down and in accordance with university of Khartoum advocacy towards e- learning, we trained teaching staff at faculty of medicine on e- learning and learning management system (LMS) through MOODLE using ZOOM. We trained teaching staff from other universities as well. Our local team visited number of universities to facilitate training of teaching staff on MOODLE.
  • We establish our own MOODLE (new version) for the EDC- faculty of medicine that can accommodate large number of participants.
  • We are preparing a manual for student guide on how to use the learning management system effectively. In addition to adding short videos to students for more elaboration.
  • We conducted a very successful workshops on research methodology and citation mangers/ educational and research technology.
